• Official transcripts: Arrange to have official transcripts sent from all colleges/universities attended. Transcripts must be sealed in an envelope bearing the official seal of the university. • Cover letter addressing your interest in Sports Nutrition and the experiences and personal qualities you would bring to this setting
• Resumé
• Three letters of recommendation (original copies of the same letters sent to the Food Science and Human Nutrition Department may be used for this purpose)

• Letters of recommendation: Three letters of recommendation that include the standard ADA evaluation form are required. Letters can be mailed by the person completing them or included in the same envelope as the other departmental application materials. All letters should be signed in blue ink, sent in sealed envelopes, and include the signature of the evaluator across the flap of the envelope.
