Application Checklist For Sports Nutrition Emphasis
Graduate School Application Materials
- Complete Application for Admission to UF Graduate School and submit by December 5, 2008. The application can be completed and submitted online or printed from online and sent to the address below. If completing the application offline, the section titled "Departmental Copy" should be mailed to the FSHN Department (see next section). (The online version is automatically sent to the Graduate School and the FSHN Department.)

- Application fee: Remit the $30.00 application fee. The online application fee can be paid online with a credit card. If the payment is made offline using a check or money order, please make it payable to the University of Florida - Office of Admissions. The application fee should be included with the application and sent to the address listed above. There are no other application fees for our program. Students completing the application online must print and sign a copy of the Signature Page and mail it with the application fee.
- Official transcripts: Arrange to have official transcripts sent from all colleges/universities attended. Transcripts must be sealed in an envelope bearing the official seal of the university. One set of transcripts should be sent to the address below. The second set should be sent to the FSHN department.

- GRE scores: Contact the Educational Testing Service to arrange to have GRE scores forwarded to the University of Florida (university code: 5812) and the Food Science and Human Nutrition department (department code: 0107). (The GRE should be taken no later than October 2008 to allow adequate time for forwarding results to the university.)
Sports Nutrition Assistantship Application
(Required for Sports Nutrition emphasis applicants only)
Sports Nutrition applicants also must submit a separate application packet to the University Athletic Association that contains the following materials:
- Cover letter addressing your interest in Sports Nutrition and the experiences and personal qualities you would bring to this setting
- Resumé
- Three letters of recommendation (original copies of the same letters sent to the Food Science and Human Nutrition Department may be used for this purpose)

American Dietetic Association Dietetic Internship Application form (available online at
http://nutrition.tufts.edu/docs/ada.doc). - Letter of Intent
- Official transcripts: Arrange to have official transcripts sent from all colleges/universities attended. Send to the address listed below. Transcripts must be sealed in an envelope bearing the official seal of the university. Sealed official transcripts also can be sent to the FSHN Department in the same envelope as the other departmental application materials.
- Letters of recommendation: Three letters of recommendation that include the standard ADA evaluation form are required. Letters can be mailed by the person completing them or included in the same envelope as the other departmental application materials. All letters should be signed in blue ink, sent in sealed envelopes, and include the signature of the evaluator across the flap of the envelope.
- ADA Verification Statement or Declaration of Intent to Complete Degree: Original copy only; a photocopy is not acceptable.

D&D Digital Computer Matching: The applicant selected for this emphasis will need to withdraw from the computer match if they have already submitted their materials to D&D Digital. Applicants not pre-selected for the Sports Nutrition emphasis will automatically be considered for the remaining internship positions if they submit the mark/sense card to D&D Digital Systems, Inc. with prioritized selections that includes our program as one of their choices by the February 13, 2009 deadline. Applicants who are not pre-selected also may apply to other dietetic internship programs through the computer matching process. Students selected for our program through computer matching have the opportunity to complete one or two four-week elective rotations (General or Research emphasis areas, respectively). Included on the list of rotations that students may request is a four-week Sports Nutrition rotation.
